Thank you for your interest in enrolling your child in Project LEAP. This registration form collects important information needed to review eligibility, confirm program placement, and ensure a safe and supportive experience for every child. Please complete all required sections accurately. Once submitted, our team will review your application and follow up with next steps within 24–48 hours.

Program Scheduling & Parental Acknowledgments

    Program Hours:
  • Early Learning (Ages 3–4): 8:00 a.m. – 6:00 p.m.
  • Afterschool (K–5): 2:30 p.m. – 6:00 p.m.
    Tuition & Fees:
  • · Weekly Program Fee: $75 per child, per week
  • I understand that tuition fees are separate from meals provided through the Child and Adult Care Food Program (CACFP) and/or the Summer Food Service Program (SFSP) and that eligible meals are provided at no additional cost.

    Nutrition Program Acknowledgment (CACFP / SFSP):
  • · BOLD Ministries participates in the Child and Adult Care Food Program (CACFP) and the Summer Food Service Program (SFSP). Nutritious meals and snacks are provided to enrolled children in accordance with USDA meal pattern requirements and served without discrimination.
 
Health & Special Needs Information

(If yes, additional medication authorization forms are required.)

 
Field Trips & Transportation Authorization
I give permission for my child to participate in supervised educational field trips, including trips to STEM-based learning centers, museums, and community institutions. I understand that transportation and supervision will be provided in accordance with DECAL safety requirements.

I certify that the information provided on this registration form is accurate and complete. I agree to comply with all BOLD Ministries – Project LEAP policies, procedures, and DECAL requirements.
